Terms of service

Provider: Sidechain HQ Ltd (trading as FormSorted), a company registered in England and Wales (company number 14661600), registered office 71-75 Shelton Street, Covent Garden, London, WC2H 9JQ. Contact: [email protected]. An independent service, not affiliated with DWP.

1. The service

We prepare written answers and documents to help you complete one of these forms or applications:

• Attendance Allowance (AA1): Claim Answers £49 or Complete Claim Pack £79 (includes the filled-in AA1)
• Disability Living Allowance for children (DLA1A): Answers £49 or Complete Claim Pack £79 (includes the filled-in DLA1A)
• PIP "How your disability affects you" (PIP2) or PIP review (AR1): PIP Answers £59 or PIP Complete Pack £79. PIP forms are sent to you by DWP, so we provide typed answers to copy across, not a filled-in form.
• Access to Work application (England, Scotland & Wales): Access to Work Answers £49 or Access to Work Complete Pack £79 (adds a support-needs summary, cost list, call prep and notes for asking for a reconsideration). Access to Work is applied for online or by phone, so we provide typed answers in application order, not a filled-in form. We don't help with disputes with an employer.

Attendance Allowance, Child DLA and PIP: England & Wales only.

Each order covers one form for one person, as described on the service page. Prices include any VAT. It's a one-off fee with no success fee. We draft your answers from what you tell us, using our own system.

2. Your responsibilities

You must give true information, check every answer, and send the claim yourself. If the form is about someone else, you confirm you're allowed to act for them: for a child, that you're their parent or look after them as a parent, or are their appointee; for an adult, that they agree or that you're their appointee or attorney. We don't estimate PIP points or predict outcomes. We can't guarantee an award or outcome; DWP or Access to Work decides.

3. Your 14-day right to cancel

Under the Consumer Contracts Regulations 2013 you can cancel within 14 days of paying, without giving a reason, for a full refund.

4. Starting the service: digital content consent

Before your claim conversation begins, we'll ask you to tick: "I want my answers prepared now. I understand that once the service starts I lose my right to cancel." If you don't tick it, you can't start until the 14 days have passed.

5. Refunds

We refund you within 14 days of your cancellation, to your original payment method. If something goes wrong with your documents, tell us and we'll fix it or refund you. This doesn't affect your statutory rights.
