Help filling in your PIP form, sorted.
Got the "How your disability affects you" form (PIP2), or a PIP review form (AR1)? Answer a guided conversation about your daily life, and we prepare clear, truthful answers for all 12 activities, the same day.
- For new claims (PIP2) and PIP reviews (AR1)
- Answers mapped to each question, with real examples and how often
- Done for you: answers, evidence checklist and a phone-call prep sheet
- One-off fee from £59. No success fee.
Your words only — you approve every word before you send.
We only use what you tell us. You see each answer next to what you said, add detail where it's thin, and approve every one. Nothing is sent by us.
How PIP claims work
- You start the claim with DWP. Call the PIP new claims line (0800 917 2222) or write to Freepost DWP PIP 1. Our Complete Pack includes a prep sheet for the call.
- DWP sends you the PIP2 form. By post (usually within 2 weeks) or as an online link by email after your call. You have 1 month to return it.
- Tell us about your daily life. One question at a time, covering all 12 activities. Short replies are fine.
- We prepare your answers. Mapped to each question, using only what you told us.
- You check and send it. Copy the answers onto your form or the online version, then send it with your evidence.
Got a PIP review form (AR1)? The same service covers it, and also notes what has changed since your last award.
The 12 activities we cover
Daily living: Preparing food, Eating and drinking, Managing treatments, Washing and bathing, Using the toilet and incontinence, Dressing and undressing, Talking, listening and understanding, Reading, Mixing with other people, Managing money. Mobility: Planning and following journeys, Moving around.
How DWP looks at each activity: what happens on most days (more than half), and whether you can do it safely, to a good standard, as often as you need to, and in a reasonable time. Our questions follow these rules so nothing important is missed.
Choose your service
PIP forms are posted or emailed to you by DWP, so we can't fill in the form itself. You get typed answers to copy or paste across.
You check and send the form yourself. Answers must be truthful. Giving false information on a benefit claim is an offence.
You can cancel within 14 days for a full refund if you haven't started your conversation. See our terms.

Common questions
Who can claim PIP?
People aged 16 or over, usually under State Pension age, with a long-term health condition or disability that makes everyday tasks or getting around difficult, expected to last at least 12 months. Over State Pension age? See our Attendance Allowance service. Check the rules on GOV.UK.
Can you help with a PIP review?
Yes. Choose "PIP review (AR1)" when you start. We map your answers to each of the 12 activity sections on the review form and summarise what has changed.
Will you tell me how many points I'll get?
No. We help you describe your situation clearly and truthfully. DWP and its assessors decide.
Is a person involved?
No. Your answers are drafted by our system from what you tell us, guided by the PIP form questions and DWP's published guidance. You review everything.
I live in Scotland
Sorry, we don't cover Scotland. There, PIP has been replaced by Adult Disability Payment from Social Security Scotland.
Are you part of DWP?
No. FormSorted is an independent service, not affiliated with DWP or GOV.UK.
What happens to my health information?
We use it only to prepare your answers, with your explicit consent, and delete it after 90 days or sooner if you ask. See the privacy notice.
